Please follow the instructions for making an appointment for the service(s) you require. All policy agreement forms require a signature, and some must be accompanied by a deposit checks. Checks should be made out to Dr. Thomas. Forms should be signed and mailed with deposits to:

45-minute Informational Consultation
If you don't have a clear idea whether you need Attention Deficit Disorder or Learning Disability testing, or if you'd like to speak with the doctor one-on-one to determine your options, you can email Dr. Thomas at nurosvcs@aol.com to schedule a 45-minute informational consultation.
To schedule an initial consultation preceding any treatment or assessment service, please email Dr. Thomas at nurosvcs@aol.com. An initial consultation is generally required before any assessment or treatment undertaking to determine the precise requirements of the case. The fee for this consultation is $175.
Full ADD, LD, Dyslexia, and/or Neuropsychological Evaluations
All full evaluations require a signed Full Evaluation Policy Agreement form (requires Adobe Acrobat Reader) and a $500 deposit. The form and check should be mailed to Dr. Thomas. You will be contacted within a week of receipt to schedule an appointment. If you are unable to download the form, please email nurosvcs@aol.com to request that one be mailed to you.
Brief ADD Evaluations require a signed Brief ADD Policy Agreement form (requires Adobe Acrobat Reader) and a $200 deposit. The policy agreement form and check should be mailed to Dr. Thomas. You will be contacted within a week of receipt to schedule an appointment. If you are unable to download the form, please email nurosvcs@aol.com to request that one be mailed to you.
QEEG Evaluation and/or EEG Biofeedback
To schedule a QEEG Evaluation or inquire about EEG Biofeedback sessions, please e-mail Dr. Thomas at nurosvcs@aol.com.
